Your medical care is provided by independent, licensed physicians and medical groups (your "Providers"). Vincera Health, LLC supports your Providers as a business associate, handling protected health information ("PHI") only as permitted by written agreements and applicable law. This notice describes how PHI may be used and disclosed in connection with your care. Treatment: sharing PHI among your Providers, pharmacies, and laboratories to coordinate your care. Payment: processing charges for services and medications. Health care operations: quality improvement, care coordination, and administrative support. Other uses — such as disclosures required by law, for public health, to avert serious threats to health or safety, or to authorities investigating abuse or neglect — are permitted or required in limited circumstances.
Marketing uses of PHI, sale of PHI, and most sharing of psychotherapy notes require your written authorization, which you may revoke at any time. We do not sell PHI.
You have the right to: inspect and receive a copy of your PHI; request corrections; request restrictions on certain uses and disclosures; request confidential communications (for example, to a specific email or phone number); receive an accounting of certain disclosures; and receive a paper copy of this notice. You will be notified as required by law if a breach of unsecured PHI affecting you occurs.
If you believe your privacy rights have been violated, you may file a complaint with us at info@vincerahealth.com or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
The terms of this notice may change; revised notices will apply to all PHI held and will be posted on this page with a new effective date.
