Vincera Health and our affiliated providers are required by law to maintain the privacy of your protected health information (PHI), provide you with this Notice of our legal duties and privacy practices, and follow the terms of the Notice currently in effect.
For treatment: Sharing PHI with your physician, care team, lab, and pharmacy to provide care.
For payment: Processing payments for services rendered.
For healthcare operations: Quality improvement, training, and administrative functions.
As required by law: Public health reporting, court orders, and similar legal requirements.
With your authorization: Any other use requires your explicit written authorization, which you can revoke at any time.
Right to inspect and copy: You may request access to your medical records.
Right to amend: You may request corrections to your records.
Right to an accounting of disclosures: You may request a list of certain disclosures we've made.
Right to request restrictions: You may request limitations on certain uses.
Right to confidential communications: You may request that we contact you in a specific way.
Right to a paper copy of this notice: Available upon request.
If you believe your privacy rights have been violated, you may file a complaint with us at privacy@vincerahealth.com or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
