For treatment: Sharing PHI with your physician, care team, lab, and pharmacy to provide care.

For payment: Processing payments for services rendered.

For healthcare operations: Quality improvement, training, and administrative functions.

As required by law: Public health reporting, court orders, and similar legal requirements.
